CARE PLUS

Care plus is a Ministry of Health funded program for people with high health needs. There are some criteria you will need to meet. For example, you may have chronic illnesses or have visited your GP or hospital services frequently over the past 12 months.


One of the aims of the Care Plus programme is to assist you to become more involved in the management of you health. 


As a Care Plus patient your first visit will involve an initial health assessment with the Care Plus nurse. This will take approximately 45 minutes. You will be involved in establishing a Wellness Plan, in conjunction with your General Practice Team. The Care Plus nurse, who will work with you on your health plan, which your GP will be supervising, will then see you every 3 months.


The Wellness Plan is designed to help you to take control of any heath conditions that could get worse over time and possibly affect your enjoyment of life. Lifestyle changes and taking medication regularly may help to prevent this from happening. With the support of your family, whanau, friends, doctors and nurses, you can take control of your health by setting some simple goals that you decide on, and achieving them. You can make a difference to staying well.
